Categories of Consumer Health Data We May Collect

Depending on what you choose to share, we may collect consumer health data that can include:

• Care needs and personal care needs (such as mobility, bathing, dressing, toileting, and eating support)
• Memory or cognitive support needs
• Medication support needs
• Behavioral concerns or safety-related needs
• Medical conditions voluntarily shared by families or representatives
• Hospital, rehabilitation, or discharge information
• Desired care setting and timeline for placement
• Location and budget preferences as they relate to care planning
• Overnight support, oxygen, hospice, or home health needs you tell us about

You are not required to provide consumer health data to receive our placement guidance. Providing it helps us better understand your situation and identify options that may be appropriate.

Purposes for Collection and How We Use Consumer Health Data

We collect and use consumer health data to:

• Respond to your request for senior living placement support
• Understand your loved one’s care needs, preferences, and priorities
• Identify possible senior living communities that may be appropriate
• Coordinate introductions or referrals to communities only with your consent
• Help families prepare for tours or conversations with communities
• Maintain business, consent, and compliance records as required by Nevada law

Sources of Consumer Health Data

We obtain consumer health data primarily from you — the person seeking placement, a family member, or an authorized representative — through our intake assessment, conversations, emails, text messages, or phone calls. In some cases, with authorization, we may receive limited information from a referring professional (such as a discharge planner, social worker, case manager, or attorney). We do not purchase consumer health data.

Categories of Consumer Health Data We May Share

With your consent, and only when reasonably necessary for placement support, we may share relevant consumer health data (such as a general summary of care needs) with selected senior living communities to evaluate possible fit, availability, services, pricing, tours, or placement options. We share only the minimum information reasonably necessary for that purpose.

We do not sell your consumer health data, and we do not use consumer health data for advertising audience segmentation or to target advertisements to you.

Categories of Third Parties Who May Receive Consumer Health Data

With your consent, categories of third parties that may receive consumer health data include:

• Senior living communities you are considering or that we refer you to
• Service providers that help us operate (such as our secure database and email-delivery providers), under appropriate data-protection terms

We maintain an internal register of vendors and processors that may access data. You may request information regarding the categories of third parties with whom your consumer health data has been shared, where required by law.

Your Rights

You may exercise the following rights. You may submit any of these requests using our Privacy & Data Request form, or by contacting Lisa directly.

• Confirm processing — request that we confirm whether we are processing your consumer health data.
• Access and information — request access to, or a copy of, the consumer health data we hold about you, and information regarding third parties with whom it has been shared where required by law.
• Correct — request that we correct inaccurate consumer health data where applicable.
• Delete — request deletion of your consumer health data, subject to legal-retention exceptions.
• Stop collecting — request that we stop collecting your consumer health data.
• Stop sharing — request that we stop sharing your consumer health data.
• Withdraw consent — withdraw any consent you previously provided.
• Stop contacting — request that we stop contacting you.
• Stop referrals — request that we stop making referrals on your behalf.
• Not be sold — you may direct that your personal information not be sold. We do not sell consumer health data.

To protect your privacy, we may need to verify your identity before processing certain requests. We will respond to verified requests within the time required by Nevada law.

Deletion Rights & Legal-Retention Exceptions

If you request deletion of your consumer health data, we will delete information that we are not legally required to retain. Certain records — such as referral disclosure and consent records required by Nevada law — may need to be retained for the applicable legal retention period. Where deletion conflicts with a legal-retention obligation, we will retain the minimum record necessary and document the legal-retention basis, and we will delete information not required to be retained.

Appeal Process

If we deny a request, you may appeal the decision by contacting us using the same methods above. We will review the appeal and communicate the result within the time required by Nevada law. If your appeal is denied, you may contact the Nevada Attorney General’s Office as permitted by applicable law.

Tracking & Technology Practices

We use basic website analytics to understand site usage. We do not place advertising, retargeting, or session-replay technologies on our intake, professional-referral, privacy request, or authenticated pages, and our analytics never capture form-field values containing consumer health data.

Policy Effective Date & Material Changes

This notice is effective as of the date listed above. If we make material changes to this notice, we will update the “Last Updated” date and, where appropriate, notify affected individuals. We encourage you to review this notice periodically.
