We are a senior living referral and placement guidance service. We are not a medical provider, healthcare facility, emergency service, law firm, financial advisor, or direct caregiving provider. For a medical emergency, call 911.

Services We Provide

Next Home Senior Placements may assist families with:

• Understanding senior living options
• Reviewing care needs and preferences
• Identifying possible assisted living, memory care, independent living, or other senior living communities
• Coordinating introductions or referrals to selected communities
• Helping families prepare questions for tours or conversations
• Providing general guidance throughout the placement process

Cost to Families

Families do not pay Next Home Senior Placements for placement services. Our guidance is free to families.

Referral Compensation

Next Home Senior Placements may be compensated by participating senior living communities pursuant to written referral agreements. If a senior living community pays a referral fee, that fee is paid by the community unless otherwise disclosed in writing. Compensation does not determine which community your family chooses, and our guidance remain focused on care needs, fit, safety, and family priorities.

We do not accept referral compensation from a community without a written referral agreement, and we do not offer gifts, bonuses, or incentives to professionals in exchange for referrals.

Consent to Contact

By submitting an intake form, calling, emailing, or otherwise requesting help from Next Home Senior Placements, you consent to being contacted by phone, text, email, or other reasonable communication methods regarding senior living placement support. You may revoke this contact authorization at any time.

Consent to Share Information

With your consent, Next Home Senior Placements may share relevant information — including a general summary of care needs — with selected senior living communities for the purpose of evaluating potential fit, availability, care needs, pricing, tours, and placement options. We share only the minimum information reasonably necessary. Your consumer health data is not sold and is not used for advertising audience segmentation.

Your Rights

• Stop contact — you may request that we stop contacting you.
• Stop referrals — you may request that we stop making referrals on your behalf.
• No sale of data — you may direct that your personal information not be sold. We do not sell your data.
• Withdraw consent — you may withdraw consent you previously provided.

Acknowledgment & Record Retention

By checking the consent box on our intake form or providing written or recorded consent, you acknowledge that you have received this disclosure (version v2.0 — August 2026), understand how our service works, and authorize Next Home Senior Placements to provide referral support. Your consent will be recorded with the disclosure version, the date and time, your name, your relationship/authority, and the consent method.

Disclosure and consent records are maintained as required by Nevada law, including for the applicable legal retention period. If you later request deletion of your information, we will delete information we are not legally required to retain, while preserving the minimum records required by law.
